MAKE A COMMENT/COMPLAINT ?
Patients wishing to comment on the services provided should contact the practice manager. We are currently setting up a Patient Participation Group, if you are interested in joining please ask Reception for details.
If you have
a complaint or concern about the service you have received from the
doctors or any of the staff working in this practice, please let us
know. We operate a practice complaints procedure as part of an NHS system
for dealing with complaints. Our complaints system meets national criteria.

OBTAIN
A HOME VISIT ?
Home visits
are at the discretion of your doctor. Please do not ask the doctor to
call unless the patient is genuinely too ill to come to the surgery.
Where the condition of the patient does require a home visit, please
let us know before 10.30am by telephoning on 8061 2197.

OBTAIN
TEST RESULTS ?
Patients
are requested to contact the surgery to check on their test results.
Patients may call in personally during surgery hours or phone after
11.00am to be given their results (Please note we are closed between 12.30 - 1.30pm Monday to Thursday and on Fridays when we are closed
between 12.30 and 2.30pm). For reasons of confidentiality we ask that the patient,
rather than a third party, phones.

MY DETAILS ?
It is important that the
practice is informed of any change of address or change of name. Also,
if you are on the telephone, please check with reception that your
number is noted on your medical records.

OBTAIN
A REPEAT PRESCRIPTION ?
If a written request for a repeat prescription is made before 11.00am then it should be available for collection the following day after 4.30pm, otherwise collect it during normal surgery times, allowing at least 24 hours. Alternatively, it can be posted to you if you provide a stamped self-addressed envelope.

REGISTER
WITH THE PRACTICE ?
Before we can register you with the practice, you need to fill in your current medical card with your new address details or, if you cannot find this, collect a registration form from reception and provide photographic ID, proof of residency and, for all patients under six years of age, a full immunisation history. We also require all patients to complete a new patient questionnaire. Newly registered patients over five years of age will be asked to see the practice nurse for a new patient's registration medical as part of the registration procedure. Please make sure that you attend for this appointment so that, should you become ill before your medical records arrive at the practice, the doctor will already have some background information.
Patients
have a right to express a preference of doctor. When you fill out the
registration forms you can indicate the doctor you would prefer. If
you are already registered with a doctor in our practice area (see map
page) and you wish to re-register with this practice, you will be
asked to attend an interview with a doctor before we accept you onto
our practice list.
